360 Degree Feedback

You may decide that 360 degree feedback is appropriate (or your employee may request it). 360 feedback is feedback gained from others such as colleagues, Members, partners or other people that your employee regularly interacts with as a part of their role.

The below form is a simple way of securing feedback. If appropriate, you should agree between you 3-5 people that it would be useful to have feedback from and to provide your employee with information secured from this exercise as a part of your overall discussion.

It is advised that responses are pooled and feedback is provided in themes rather than referring to individual comments verbatim therefore confidentiality can be maintained.
