The ICO have an updated reporting form which can be used for any future personal data breaches. The form can be found on the GDPR Molly page under GDPR Documents and Forms.
Please complete as much detail as you can and then forward to Tom.Penlington@molevalley.gov.uk who will submit the report to the ICO.
