Further free flu jabs are able to be offered to staff this year. If you have not yet received a flu jab and would like one please contact Medwyn by emailing admin@medwynoh.co.uk or telephone 01306 873 936 to book an appointment with them directly.

Please ensure you complete the attached consent form below and take this with you to your appointment.
Please make sure you attend your appointment and arrive at least 5 mins before.
If you need to cancel your appointment, please cancel your own appointment by contacting Medwyn directly via the above email or telephone number.
Please be aware that the flu vaccine being offered via this clinic is different to the vaccine offered to adults aged over 65. If you are over age 65 please book your flu vaccine through your GP to ensure you receive the correct jab.
